A Proof of Residency Letter is a written sworn statement that declares an individual resides at a certain address, typically to prove state residency. 

The letter is a type of affidavit that is to be considered to be an official statement. You must sign the document along with witnesses that can verify your address.

Forms of identification like a copy of your driver’s license can also be used. It would also be a good idea to ask for a notarized letter from your landlord confirming tenancy and months of residency if you rent an apartment.

1. Add your personal information: As the person giving the sworn statement, you’ll need to provide your name, social security number, birthday, address, and how long you’ve resided at the address.
2. Include your witnesses: Anyone that lives in the same state as you who isn’t your spouse can be a witness to you signing the document. You have to list them and state what your relationship is with each person that you list.

5. Notarize the letter: The letter must be notarized by a notary public to be considered official.
