A Proof of Residency Letter is a written sworn statement that declares an individual resides at a certain address, typically to prove state residency. 

The letter is a type of affidavit that is to be considered to be an official statement. You must sign the document along with witnesses that can verify your address.

1. Add your personal information: As the person giving the sworn statement, you’ll need to provide your name, social security number, birthday, address, and how long you’ve resided at the address.
